From top to bottom...
1) Sunglasses
2) Shoulder gun holsters
3) Black long sleeved t-shirt
4) Black floor length trench coat.

5) Standard issue Black military BDU's
6) Thigh, waist and belt gun holsters.
7) Airwalk boots w/buckles (see below)
Neo's Boots UPDATE

Thanks to some research done by Neo616 of the Matrix message boards, the boots worn by Neo in the lobby and roof scene were not made by Airwalk. However when the were made Airwalk apparantly paid big money to put their logo on there. (Smart move by airwalk-they obviously knew how well the movie would do) There is a replica version veing sold at www.airwalk.com. Check it out for yourself.
It should be warned that they are VAGUELY similar to the ones Neo wore. They do not have the chain-mail toes, nor are they as tall either. You be the judge for comparison. See the pictures below. The one on the left is the official boot ACTUALLY worn by Keanu in the film (click for larger pic with detail.) The one on the right is the airwalk replica.
